Dothan Preparatory Academy New Student Enrollment

In order to enroll your student at Dothan Preparatory Academy you will need:

• Social Security Card
• Birth Certificate
• Withdrawal Forms from previous school
• Proof of Residency (two forms)
• Current Alabama Immunization Record (Blue Form)
